Initial Nutrition Assessment

60 minutes

Prior to the appointment, you will complete an intake form outlining your medical, nutritional, and lifestyle history.

During the assessment, the Dietitian will review the details in your intake form, discuss your expectations, identify your short-term and long-term goals, and consider potential areas for growth.

Together, you and the Dietitian will develop a personalized nutrition plan. The Dietitian will suggest some recommendations, which may include nutrition education, worksheets, recipe cards, or handouts, based on your needs.

*Required for new clients.

Follow-Up Session

35 minutes

During the follow-up session, The Dietitian will review your current progress with the nutrition plan.

Each session is an opportunity to explore your strengths or any challenges encountered to further support your food journey.

Making lifestyle changes and reconnecting with food takes time. The Dietitian will readjust the nutrition plan as needed and suggest modifications, at a pace that is comfortable for you.

Diet Review + Analysis

35 minutes

You will complete a 3-day food record that will be analyzed by the Dietitian to generate a personalized nutrient intake report which includes a breakdown of the 13 primary nutrients:

  • Energy value in Calories, along with the daily fat, saturated fat, trans fat, carbohydrate, fibre, sugars, protein, cholesterol, sodium, potassium, calcium, and iron intake. Other micronutrients can be included in the analysis if requested (extra charge of $5 per nutrient).

You will receive a digital report of the diet analysis and the results of the diet review will be discussed during your next follow-up appointment.

Based on the data, we will identify your dietary strengths, meal timing, and find areas for improvement to make sustainable and healthy changes to your eating.

Individualized 7 Day Meal Plan

The Dietitian will create an individualized 7-day meal plan and grocery list based on your medical and nutrition history. The meal plan will reflect your Energy, Macronutrient, and Micronutrient needs, as well as consider your personal schedule, physical activity levels, food preferences, food planning, preparation & cooking skills, food affordability, and food accessibility.

Each meal plan will be curated with your needs in mind.
